Class Record Forms

 

Below are the Class Record Forms for the New DPMT and New DPRT for Levels 1 – 6. Test users may wish to record scores on the Class Record Form to generate a record of the performance of all pupils in a class. Since errors may occur in transferring scores to the Class Record Form, the accuracy of transferred scores should be checked.

 

For each Level there is one form for Level 1 (First class), one for Level 2 (Second class) and one for Levels 3-6 (Third to Sixth classes). For both tests and at each level, the form is available as a Word document (for electronic editing) and as a pdf.
